Why is Apple Tree doing research?

Why do I need to sign a consent form?

  • Apple Tree wants you to know that its data is used for research.
  • Minnesota is the only state that requires permission to use patient records for research. (This is outlined in Minnesota statute 144.295.)

How does Apple Tree protect my privacy?

  • Apple Tree Dental is always required to protect the privacy of your health information, whether or not you give permission for it to be used for research.
  • Apple Tree Dental and its research partners follow applicable privacy laws. Your name and other identifying information are NEVER part of research publications.
  • When applicable, research projects are approved by the Institutional Review Board (IRB). The IRB is a group of doctors, nurses, and citizens that review each research study to ensure that patient information is kept private and protected.

Can I change my mind about whether my information is used for research?

  • You can change your mind at any time.
