Apple Tree wants to help individuals who don't have dental insurance and can't afford the full cost of care by offering a simple way to lower the fees and help you receive the dental care you need.
It's not another type of payment plan. This is a program that reduces the fees you will be charged overall, based on your income and family size.
Eligibility is based on your income and the number of people in your family. Seniors aged 65 and older without insurance coverage automatically receive a 10% discount, but may qualify for up to 30% by applying for the Affordable Plan. To determine if you qualify, fill out a one-page application and submit income documentation. Apple Tree staff will determine which Affordable Plan you are eligible for.
In order to receive the Affordable Plan discount, all payments are due at the time of service.
